Requesting Medical Records

To obtain medical records for services provided by Nemours or to receive copies of an X-ray, please print and complete the Authorization to Release Form. You can return the form by:

  • The processing time for copies of records is 8-10 business days.
  • For personal copies of records, a flat rate of $6.50 will be assessed (CD, email, fax or mail).
  • Please call (866) 956-7299, press option #1, to check the status of your request.
